If you have experienced an issue with a medical receptionist or physician and wish to report it formally.

When form is received via mail or email (response will be returned within five business days).

Medical records will be kept onsite until transfers are requested. 

To fill out Authorization for Release of Medical Information and/or Medical Records, see link below. Form will need to be completed and a fee of $25 to be collected prior to release of records.

As this is an uninsured service by Manitoba Health. There is a $25 minimal charge concerning the copying of medical records to be incurred by the patient as per Doctors Manitoba Fee Guidelines for Uninsured Health Services.

Uninsured Services are services provided  that are not insured by the provincial health insurance plan. These include items  such as medical reports requested by insurance companies, medico-legal reports, transfer and access of medical records, sick notes, and driver’s medicals. It may also include services provided to individual who are not insured such as those who are not Canadian residents.

Doctors Manitoba has provided guidelines on these uninsured services.
